Our Service Model encapsulates our commitment to individual needs of people with disability, including people with psychosocial disability, and their families and carers by enhancing choice, sustainability and flexibility.

Our methods combined then enable Sunflower Care Services to establish a dynamic; person centred Individual Care Plan (ICP) which focusses on two areas:

• the support required by the Individual
• their behaviours, skills, values or preferences

Each phase of our Service Model is underpinned by engagement, compliance with rules, regulation and law and our quality and continuous improvement.
